Job summary

Central and North West London NHS Foundation Trust is seeking an experienced clinician to provide specialist exercise and cardiac rehabilitation support within the Community Cardiac Rehabilitation Programme. You will conduct holistic assessments, develop evidence-based, patient-centered exercise plans and work closely with patients, carers and the multidisciplinary team to optimise health and independence.

• Advanced clinical reasoning and holistic assessment skills
• Strong communication with patients, carers and MDT
• CPD to maintain specialist knowledge in cardiac rehabilitation and exercise therapy
• Ability to contribute to service development and quality improvement

Responsibilities

• Deliver autonomous, evidence-based cardiac rehabilitation within the Community Cardiac Rehabilitation Programme.
• Assess, diagnose and manage risk for complex cardiovascular conditions.
• Create personalised exercise and physical activity plans and support behaviour change.
• Collaborate with multidisciplinary teams to coordinate care and referrals.
• Support audits, research and data collection to improve patient outcomes.
