Job summary

Central and North West London NHS Foundation Trust is seeking a physiotherapist to deliver autonomous, evidence-based cardiac rehabilitation within the Community Cardiac Rehabilitation Programme. You will assess patients with cardiovascular disease, design personalised exercise and activity plans, educate patients and carers, and work with the multidisciplinary team to ensure safe, effective care.

You will maintain records, participate in CPD, contribute to service development, quality

Qualifications

• Degree in Physiotherapy
• Degree in Sport and Exercise Science or Exercise Physiology
• BACPR Exercise Instructor REPs Level 4
• Strong verbal and written communication skills
• Valid car driver
• Advanced skills in assessment and treatment of adults with complex multi-pathologies
• Advanced knowledge and understanding of the cardiac rehab process.

Responsibilities

• Provide autonomous, evidence-based cardiac rehabilitation for patients with complex cardiovascular conditions.
• Assess, diagnose and manage risk, delivering personalised exercise plans with advanced clinical reasoning.
• Collaborate with the MDT to ensure safe, effective patient-centred care and coordinate referrals.
• Maintain accurate records, contribute to service development, CPD, audits and teaching.
