Frimley Health NHS Foundation Trust in Slough is seeking an enthusiastic Cardiac Disease Risk Management Exercise Professional to join our multidisciplinary team at Wexham Park Hospital. You will support patients recovering from cardiac events and cardio-respiratory conditions by guiding safe, effective, individualized exercise programmes.

Responsabilités

• Exercise prescription: Designing safe, appropriate exercise programmes for patients with cardiac and cardio-respiratory conditions.
• Exercise delivery: Supervising and delivering exercise sessions as part of the multidisciplinary team (MDT).
• Patient assessment: Taking part in initial and final assessments of patients entering and completing the cardiac disease risk management programme.
• Clinical safety: Ensuring exercise is appropriate for patients recovering from cardiac events and those with existing cardio-respiratory disease.
• Evidence-based practice: Keeping exercise prescription aligned with current research, guidelines and best practice.
• Audit and research: Participating in clinical audit, research and evidence-based improvement projects.
• Service improvement: Helping introduce changes to practice and monitoring whether standards and outcomes are maintained.
