Job summary

Qualifications

• BACPR certified cardiac rehabilitation knowledge.
• Experience in leading exercise-based rehab sessions.
• Ability to assess patients and tailor programmes.

Responsibilities

• Support patients in the immediate recovery following a cardiac event within phase 3 rehabilitation with multi-disciplinary team input.
• Develop and deliver in-person and telephone-based exercise and education programmes; assess participants joining these programmes.
• Act as an autonomous practitioner, planning and developing appropriate treatment programmes tailored to individual needs.
• Lead delivery of the exercise component of phase 3 rehabilitation classes for complex or chronic cases.
• Monitor, evaluate and record patient outcomes; provide high-quality patient care with individual goals considering cardiovascular risk factors.
• Report to enhanced practitioners and service lead; support the development of junior staff and students.
