Job summary

Qualifications

• Lead on discharge coordination and patient flow within CAMHS.
• Provide clinical oversight for complex discharge cases.
• Collaborate with multidisciplinary teams and external agencies.
• Record and monitor care pathways and reporting.
• Assess safeguarding and contribute to risk management.
• Develop evidence-based practice and monitor service quality.

Responsibilities

• Lead on the coordination of patient flow across the Lewisham CAMHS pathways.
• Identify barriers to discharge and implement solutions with teams.
• Provide advice on recording care pathways on epjs and reports.
• Monitor caseloads, length of stay and service capacity.
• Liaise with external agencies to ensure smooth transitions and continuity of care.
• Provide clinical oversight and support to practitioners handling complex cases.
