Job summary

West London Mental Health Trust is seeking a motivated Discharge Coordinator to join the Hospital Discharge Team. You will manage a caseload of patients with varying complexity and coordinate safe, timely discharges from admission through to discharge.

Working closely with wards, community services, social care and external partners, you will remove barriers to discharge, lead on complex discharge planning, and promote best practice to improve patient outcomes and experience.

An exciting opportunity has arisen for a motivated and experienced Discharge Coordinator to join our dynamic Hospital Discharge Team.

You will play a key role in supporting patient flow and coordinating safe, timely discharges from admission through to discharge. Managing a caseload of patients with varying levels of complexity, you will work closely with multidisciplinary teams, community services, social care, and external partners to remove barriers to discharge and reduce length of stay.

The successful candidate will lead on complex discharge planning, support pathway coordination, facilitate access to specialist equipment and community services, and act as a key escalation point to ensure patients are discharged safely and without unnecessary delay.

You will also provide leadership and support to discharge coordinators, promote best practice, and contribute to service improvement initiatives.

Key Responsibilities

• In liaison with ward staff, community, social services and other healthcare colleagues facilitate timely, appropriate and safe discharge of patients.
• Attend Daily Reason to Reside Meetings, MDT meetings, Professional and family meetings, Daily Board rounds on allocated wards.
• Ensure that the Early Discharge Notification (EDN) to be completed within 24 hours of hospital admission, Needs Based Assessment (NBA) and any other onward referrals are completed in a timely manner.
• Ensure the person and any carer’s, family or friends are involved in and informed about all aspects of the discharge process.
• To promote and ensure that early bed release guidelines are adhered to; ensuring that all appropriate patients are moved to the discharge lounge.
• Promote nurse led discharges at weekends and assist on competency levels to empower staff both at ward level and within the discharge team.
• Actively support and facilitate the transfer of patients who are privately funding their care to prevent extended length of stay. To adhere to the National and Trust Policy for Discharge to family and carers within the agreed time frame to avoid extended length of stay.
