Hackney Council is seeking a Brokerage Officer with experience in Adult Social Care brokerage, commissioning or placement finding. You’ll source, arrange and amend packages of care across residential, nursing homes and supported living services, supporting adults including older people, learning disabilities and mental health service users.

• Experience in health or social care environments.
• Experience of residential or supported living placements.
• Strong stakeholder and provider relationship management skills.
• Confident negotiation and influencing skills.
• Experience using IT, administrative, contact management or case management systems.
• The ability to manage a busy workload, prioritise effectively and meet deadlines.
• Experience working with external agencies and stakeholders.
• An understanding of commissioning, procurement and achieving value for money within care services.

• Supporting supplier set-up, contracting and amendments
• Working closely with Care Management, Hospital Discharge, Finance and Information & Assessment teams
• Maintaining accurate records and updating Mosaic and CM2000
• Supporting the identification of service gaps and potential new providers
• Maintaining up-to-date knowledge and intelligence of the local care market
• Contributing to commissioning and market management activity
• Supporting person-centred services that promote choice, independence and positive outcomes.

This is a great opportunity for someone with experience within Adult Social Care brokerage, commissioning or placement finding who is confident working with care providers and internal teams to secure high-quality, value-for-money care and support., As a Brokerage Officer, you'll work closely with Service Managers, Senior Practitioners and Care Managers to source, arrange and amend packages of care.
