Job summary

Responsibilities

• Assist residents with personal care per individual care plans.
• Liaise with home management, families, social workers, and medical professionals.
• Administer and document medications safely per policy.
• Monitor nutrition and update dietary plans as needed.
• Monitor for pressure injuries and urinary output changes.
• Support and promote resident activities.
