Central and North West London NHS Foundation Trust is seeking an experienced Registered Nurse / Advanced Clinical Practitioner to join the Virtual Ward Senior Practitioner team in Milton Keynes on a fixed-term contract.

You will deliver advanced clinical assessment, coordinate patient pathways, and work across the wider health system to avoid hospital admissions, with flexible, patient-centred care and access to Doccla remote monitoring.

Qualifikationen

• Current UK professional registration is required.
• Advanced assessment skills and history taking.
• Ability to drive and access to a car is essential.

Aufgaben

• Undertake advanced clinical assessment of Virtual Ward patients and develop care plans.
• Maintain high level clinical skills including IV administration, cannulation, ECGs where required.
• Ensure delivery of high-quality care across the Virtual Ward and across health system.
• Lead MDTs and develop clinical pathways with MKUH and SDEC where appropriate.
• Support development of national initiatives related to Virtual Ward services.
• Coordinate patient pathways and ensure accurate information at regular clinical meetings.
• Monitor patients remotely using Doccla across respiratory and frailty pathways.
• Ensure effective referral pathways and initial triage for Virtual Ward patients.
• Oversee caseload management and ensure patient-centered care.
