Job summary

Locala Community Partnerships CIC is seeking a Band 6 Senior Community Nurse to join our North Kirklees Community Nursing Network. This full-time, 37.5-hour post offers a leadership role delivering person-centred care across North Kirklees and Greater Huddersfield.

You will combine clinical expertise with line-management duties, mentoring Band 5 nurses, leading on service development and working between 8am and 9pm with weekend and late shift patterns.

Qualifications

Responsibilities

• Lead and coordinate care for patients with complex needs, ensuring high standards of clinical practice.
• Provide guidance, supervision and mentorship to Band 5 nurses and wider team members.
• Delegate tasks effectively to ensure efficient use of resources and timely delivery of care.
• Support service development initiatives and contribute to quality improvement projects.
• Act as a point of escalation for clinical decision‑making and safeguarding concerns.
• Promote self‑care and independence for patients living with long‑term conditions.
• Collaborate with multidisciplinary teams to prevent unnecessary hospital admissions.
• Provide operational and clinical leadership in the absence of the Team Leader, supporting the coordination of staffing, workload and service delivery.
• Act as a positive role model, helping to create a supportive team culture focused on quality, learning and continuous improvement.

An exciting opportunity has become available within the North side of Kirklees for an experienced, motivated and compassionate Registered Nurse to join our Community Nursing Network as a Band 6 Senior Community Nurse (37.5 hours per week, full time).

This is an excellent opportunity to take on a leadership role, supporting our teams to deliver exceptional care across North Kirklees and Greater Huddersfield.

Working Pattern

This is a full‑time position of 37.5 hours per week. Our service operates between 8am and 9pm, with core hours of 8am to 5pm. Successful candidates will be expected to work a minimum of one weekend in four and undertake two late shifts (1pm to 9pm) within a four‑week period.

This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK.
