Job summary

Locala Health and Wellbeing in the North Kirklees area is seeking an experienced Band 6 Senior Community Nurse to lead our Community Nursing Network. This role combines clinical leadership with mentorship across the team and requires a proactive approach to service improvement.

You will provide clinical leadership, mentor Band 5 nurses and drive patient outcomes while supporting operational delivery across North Kirklees and Greater Huddersfield.

Qualifications

• Registered Nurse with substantial community nursing experience and a passion for leadership.

Responsibilities

• Lead and coordinate care for patients with complex needs, ensuring high standards of clinical practice.

• Support service development initiatives and contribute to quality improvement projects.
• Act as a point of escalation for clinical decision-making and safeguarding concerns.
• Promote self-care and independence for patients living with long-term conditions.
• Collaborate with multidisciplinary teams to prevent unnecessary hospital admissions.
• Provide operational and clinical leadership in the absence of the Team Leader, supporting the coordination of staffing, workload and service delivery.

An exciting opportunity has become available within the North side of Kirklees for an experienced, motivated and compassionate Registered Nurse to join our Community Nursing Network as a Band 6 Senior Community Nurse.

This is an excellent opportunity to take on a leadership role, supporting our teams to deliver exceptional care across North Kirklees and Greater Huddersfield.

As a Band 6 Senior Community Nurse, you will combine clinical expertise with leadership skills to ensure safe, effective and person‑centred care. Working Pattern

This is a full‑time position of 37.5 hours per week. Our service operates between 8am and 9pm, with core hours of 8am to 5pm. Successful candidates will be expected to work a minimum of one weekend in four and undertake two late shifts (1pm to 9pm) within a four‑week period.

This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK.
