Job summary

Band 6 Primary Care Mental Health Practitioner role within the West Birmingham Hub of Birmingham and Solihull Mental Health NHS Foundation Trust. The post involves working in the Neighbourhood Mental Health Team to deliver triage, assessment and outcomes-based interventions for local communities.

You will collaborate with GPs and primary care professionals in a multidisciplinary setting, driving care pathways and safeguarding compliant care as part of the BSOL transformation programme.

Qualifications

• Registered Mental Health Nurse/OT/social worker
• Post-registration experience in mental health services
• Strong assessment and risk management capabilities; leadership potential

Responsibilities

• Provide mental health triage, assessment and outcomes-based intervention.
• Identify needs and pathways for service users and carers.
• Maintain robust communication across mental health providers and primary care and support PCN colleagues.
• Support multidisciplinary Neighbourhood MDT collaboration.
