Job summary

Birmingham and Solihull Mental Health NHS Foundation Trust in West Birmingham is recruiting a Band 6 Primary Care Mental Health Practitioner to strengthen the Neighbourhood Mental Health Team within the PCNs. You will work alongside GPs and other primary care professionals to deliver triage, assessment and evidence-based interventions in the community.

Qualifications

• Registered professionals with mental health registration including nursing, OT or social work.
• Relevant post registration experience in mental health.
• Understanding of Care Programme Approach, Clinical Governance, Risk Assessment & safeguarding.

Responsibilities

• Provide mental health triage, assessment and outcomes-based intervention within the Neighbourhood Mental Health Team.
• Identify needs and advise on best course of support or intervention, promoting psychosocial wellbeing.
• Ensure robust communication across mental health providers, VCSE organisations and primary care, and support PCN colleagues.
