Job summary

Kent Community Health NHS Foundation Trust is seeking a Band 6 Lead Pharmacy Technician to join the Frailty Urgent Care Services, delivering hospital-level care to patients in their homes.

The role aims to reduce hospital admissions, support earlier discharge, and ensure medicines are used safely throughout the care journey. It involves remote and face-to-face reviews as part of a multidisciplinary team.

Qualifications

• Registered Pharmacy Technician with NHS/community experience.
• Experience in frailty care and home-based medicines management is desirable.
• Ability to lead and coordinate a multidisciplinary team.

Responsibilities

• Medicines reconciliation, review and counselling, remotely and face-to-face.
• Attendance at multidisciplinary board round.
• Assess medication adherence and implement interventions.
• Identify and refer priority patients for pharmacist review.
• Independently resolve medicines-related queries and escalate as needed.
• Liaise with stakeholders during transfer of care to support medication management.
• Oversee medication stock management and supply.
