Job Description:

The team in Rayleigh are looking for a dedicated Community Psychiatric Nurse to join them to deliver high quality integrated health and social care for individuals facing mental health issues. You will work closely with service users to build effective partnerships, providing personalised care and support tailored to their needs and preferences.

Your role will include, but is not limited to:

• Maintaining high standards of care while respecting service users' individuality and dignity, promoting independence.
• Ensuring each patient receives an individual assessment and regularly updated care plan, formulated with the service user, in line with the Trust’s CPA Care Coordination Policy.
• Communicating information regarding the service user’s needs and progress promptly and accurately to relevant parties, maintaining confidentiality.
• Applying a Behavioural Family Therapy approach to care when appropriate.
• Facilitating open communication within the team and with other agencies to promote multidisciplinary care.
• Working as CPA Care Coordinator for service users as required.
• Encouraging positive mental health among service users.
• Providing psychiatric nursing interventions, psychological care, and advice.
• Participating in a duty system regularly.
• Ensuring a recovery-focused approach to patient intervention.

Requirements:

• Minimum of 6 months experience in a similar role.
• Experience with CMHT.
• Degree relevant to NMC registration and current NMC registration.
• Experience of taking responsibility.
• Experience supporting practice development.
• Enhanced DBS check (preferably on the update service) to expedite recruitment.
• Mandatory training certificates.
• Full driving license and access to own vehicle are advantageous.
