Job summary

East London Community Forensic Service (ELCFS) is an enhanced multidisciplinary service providing care, treatments, and support to service users in the process of and following discharge from secure care services. ELCFS also provides a liaison and consultation service to secondary care mental health services. ELCFS serves/cover both ELFT (Newham, City and Hackney, and Tower Hamlets) and NELFT (Barking & Dagenham, Havering, Redbridge, and Waltham Forest) localities.

An exciting opportunity has arisen for an experienced and motivated band 7 registered mental health nurse to join our expanding service.

This role offers the successful candidate the chance to work within NHS England's innovative 'specialist community forensic team model,' which focuses on:

  1. Providing INREACH support to help service users move safely towards discharge, reducing length of stay in secure care.
  2. Providing enhanced support in the community to promote sustained recovery and quality of life, reducing hospital recalls.

The successful candidate will manage a caseload of service users discharged from forensic inpatient services into the community. You will act as a care coordinator for these service users in a designated locality and collaborate with relevant agencies, services, families, and carers.

The postholder will be expected to serve one or more of the localities covered by the service (e.g., Newham).

Main duties of the job
  • Care coordination of locality forensic patients discharged into the community as agreed by the MDT sector team.
  • Conduct comprehensive specialist forensic nursing assessments of mentally disordered offenders in various complex settings related to forensic or community mental health teams.
  • Plan, implement, and evaluate care programmes aimed at reducing violence and re-offending.
  • Engage in quality improvement projects, using systemic thinking to develop and test change ideas within the Trust's QI framework.
  • Administer medication safely following relevant policies and procedures.
  • Represent forensic community mental health services at Multi-Agency Public Protection Meetings and/or local Mentally Disordered Offenders Groups.
  • Provide specialist advice on managing mentally disordered offenders in the community, including risk assessment and management, and support transfer and care processes.
  • Develop care packages and psycho-education for offenders and their families/carers, considering family systems and social networks.

Person Specification

Education/Qualification/Training (Essential):

  • RMN Part III of the NMC Register
  • Relevant Post Graduate qualification in Forensic Practice, Mental Health, or Community Mental Health Nursing (e.g., ENB A71, Diploma, ENB 650, Appropriate Degree)

Additional requirements include:

  • Evidence of working towards an MSc or similar qualification.
  • HCR-20 risk assessment (or willingness to complete).
  • ENB 998/Mentorship course.

Desirable:

  • PCL-R, SVR Risk Assessments.
  • Dual Diagnosis training.

Experience (Essential):

  • Relevant experience at Band 6 or above in a forensic setting.
  • Experience working with offenders and patients with complex needs (e.g., MDOs, Personality Disorder, Learning Difficulties).
  • Inter-agency working experience.

Knowledge and Skills (Essential):

  • Skilled in nursing assessments, care planning, and packages of care.
  • Knowledge of M.H.A. 1983 and CPA framework.
  • Understanding of Criminal Justice Act and multi-agency collaboration.

Additional desirable skills include problem-solving, policy development, teaching, and knowledge of treatment models.
