Bradford District Care NHS Foundation Trust is seeking Band 6 Community Care Coordinators to join our City Community Mental Health Team in Bradford. You will manage defined caseloads, assess, plan, intervene, and evaluate care to deliver person-centered mental health support.
We encourage flexible working and offer a range of benefits. Applicants must have UK right-to-work status and relevant professional registration.
Competenze
- Essential criteria: registration with a professional body (e.g. NMC)
- Minimum 6 months post-basic experience in Mental Health
- Experience of working in a community mental health setting
- Desirable criteria: experience of being clinical adviser to junior staff or students
Mansioni
- Coordinate and deliver community mental health care in line with Trust policies
- Undertake comprehensive assessments and manage risk effectively
- Work collaboratively with service users, carers, and partner agencies
- Administer and monitor medication in accordance with NMC and Trust guidelines
- Participate in case conferences and contribute to service development
- Promote equality, diversity, and safeguarding in all aspects of care
