Zusammenfassung

Bradford District Care NHS Foundation Trust is expanding its City Community Mental Health Team and seeking motivated Band 6 Community Care Coordinators to join the team in Bradford. You will manage a defined caseload, deliver high-quality, person-centered mental health care, and work autonomously within a multidisciplinary team.

The role involves assessment, planning, intervention, and evaluation of care to ensure safe, effective, and recovery-focused support.

Qualifikationen

• Essential: Professional qualification in mental health nursing, social work, occupational therapy.
• Essential: Registration with Professional Body (e.g. NMC, SWE, BAOT).
• Experience: Minimum of 6 months post basic experience in Mental Health.
• Experience: Experience of working in a community mental health setting.
• Desirable: Experience of being clinical adviser to junior staff or students.

Aufgaben

• Coordinate and deliver community mental health care in line with Trust policies.
• Undertake comprehensive assessments and manage risk effectively.
• Work collaboratively with service users, carers, and partner agencies.
• Administer and monitor medication in accordance with NMC and Trust guidelines.
• Participate in case conferences and contribute to service development.
• Promote equality, diversity, and safeguarding in all aspects of care.
