Bradford District Care NHS Foundation Trust is expanding its City Community Mental Health Team in Bradford, seeking motivated Band 6 Community Care Coordinators. You will manage a defined caseload, work autonomously within a multidisciplinary team, and deliver high-quality, person-centered mental health care to local service users.

Responsibilities include assessment, planning, intervention, and evaluation of care, with a commitment to safeguarding, equality, and recovery-focused support.

Responsabilités

• Coordinate and deliver community mental health care in line with Trust policies.
• Undertake comprehensive assessments and manage risk effectively.
• Work collaboratively with service users, carers, and partner agencies.
• Administer and monitor medication in accordance with NMC and Trust guidelines.
• Participate in case conferences and contribute to service development.
• Promote equality, diversity and safeguarding in all aspects of care.
