Responsibilities

• Provide high-quality, person-centred care to patients in their own homes and community settings.
• Perform assessment and care planning, wound management and support for long-term conditions.
• Deliver palliative and end-of-life care.
• Collaborate with multidisciplinary health and social care professionals to coordinate care.
