The Maidenhead Primary Care Network (PCN) is seeking a Paramedic to work as an autonomous practitioner within a multidisciplinary team. You will provide same day and planned care for patients in the practice and community, including home visits and support for care homes.

Essential requirements include an undergraduate degree in Paramedic Science, full HCPC registration, and a valid UK driving licence. The role is permanent, part-time with a salary of £42,686–£52,000 per year FTE, pro rata for

• Undergraduate degree in Paramedic Science or equivalent Level 6 qualification.
• Full HCPC registration as a Paramedic with no cautions or restrictions.
• Valid UK driving licence.

Responsabilidades

• Assess, diagnose, treat and refer or signpost patients with undifferentiated or undiagnosed conditions.
• Provide same day and planned home visits including urgent assessments.
• Support care homes with acute visits, rounds and MDT meetings.
• Request and interpret investigations such as ECGs and blood tests per local guidance.
• Contribute to anticipatory and personalised care planning for long-term conditions and frailty.
• Promote self-care and signposting to social prescribing and community services.
• Maintain accurate clinical records and participate in audit and quality improvement.
• Collaborate with the wider MDT and attend PCN meetings.
