Job summary

Hodnet Medical Centre is seeking an Advanced Nurse Practitioner to autonomously assess, diagnose, and manage both acute and chronic conditions, working closely with GPs and the wider multidisciplinary team. The post focuses on expanding caseload with mentoring and support for blood results management, within a rural dispensing practice of 4,200 patients.

Qualified Advanced Practitioner with clinical leadership skills.
NMC registered and able to prescribe independently.

Responsibilities

Autonomous assessment, diagnosis, treatment and ongoing management of acute and long-term conditions.
Assess and treat patients in surgery or at home needing acute care, referring or admitting to secondary care as needed.
Refer patients to specialties and multidisciplinary teams, including for imaging and testing.
Make independent decisions and direct referrals to hospital consultants as appropriate.
Support and advise colleagues when standard protocols do not apply.
Act as extended and supplementary prescriber within scope of practice.
Interpret tests and follow up test results to ensure continuity of care.
