Job summary
South Tyneside and Sunderland NHS Foundation Trust is seeking a Junior Nurse Practitioner to join the Community Frailty Team in Sunderland. You will autonomously assess and coordinate care for older adults with frailty, performing CGAs and care planning while collaborating with a multidisciplinary team.
The role requires an experienced registered nurse with NMC, and a willingness to develop advanced prescribing and assessment skills. Driving licence and car access are preferred.
Qualifications
- Registered Nurse with NMC registration.
- Mentorship certificate or demonstrable equivalent experience.
- Non Medical Prescriber or willingness to complete.
- Advanced clinical assessment skills or willingness to work towards.
- Full UK driving licence and access to a car.
- Experience in care of older people and frailty.
Responsibilities
- Coordinate the delivery and development of the specialist frailty service to national standards.
- Undertake CGAs and personalised care planning to support self-care and reduce frailty.
- Lead practitioner-led clinics, support hospital discharge, and optimise treatment plans.
- Provide education and training to clinical staff across the Trust.
- Represent the service nationwide and ensure targets are met.
