Job summary

South Tyneside and Sunderland NHS Foundation Trust is seeking a Junior Nurse Practitioner to join the Community Frailty Team in Sunderland. You will autonomously assess and coordinate care for older adults with frailty, performing CGAs and care planning while collaborating with a multidisciplinary team.

The role requires an experienced registered nurse with NMC, and a willingness to develop advanced prescribing and assessment skills. Driving licence and car access are preferred.

Qualifications

Junior Nurse Practitioner

We are looking for a highly motivated NursePractitioner to work within the Sunderland Community Frailty Team inSunderland. The team consists of Band 7 and Band 6 Nurse Practitioners who arebased in each of our six Community Integrated Teams. You will be responsiblefor out of hospital care assessment and co-ordination of care and services topeople who live in their own home or nursing or residential care and areidentified as having moderate to severe frailty, multiple co-morbidities complex needs or who are identified as high intensity service users. You will work as a member of theMultidisciplinary Team and will be required to attend and participate care homeand community MDTs. You will be an autonomous practitioner.

You will be responsible for pro-actively case finding patients with moderateto severe frailty and undertaking CGAs Comprehensive assessment, personalisedcare planning, providing support and advice to enhance self-care and improvequality of life for older people and reducing level of frailty whereverpossible.

You will work autonomously, assessing individualpatient needs, initiating investigations, with support determining adifferential diagnosis and initiating appropriate holistic, evidence-basedtreatment and care. You will refer patients who require medical intervention tothe appropriate services.

Main duties of the job

As a JuniorNurse Practitioner in the Community Frailty Team you will be an experienced Registered Nurse, whopreferably has experience at Band 6 level, you will have a great workingknowledge of care of older people and patients with frailty and complex healthneeds and you will be a confident autonomous professional. You will be adynamic, approachable and self-motivated individual, youwill be responsible for ensuring high standards are maintained and you will havea desire for continual improvement.

You will have:

A full UK driving licence and access to a car

A nursing or related degree

Advancedclinical assessment skills accreditation or willingness to work towards

Non-medicalprescribing qualification or willing to work towards extended prescribingskills

Knowledge & Skills
