Job summary

NHS Greater Glasgow and Clyde is seeking an Advanced Frailty Practitioner to lead advanced assessments and care coordination for frail older adults. The role works across health and social care with a strong focus on rehabilitation and prevention of hospital admissions.

Qualifications

• Registered nurse or Allied Health professional with significant frailty experience.
• Master's level qualification in Advanced Practice, or working towards this.
• UK driving licence and vehicle for work purposes.

Responsibilities

• Conduct advanced clinical assessment and decision making for frail older adults.
• Develop personalised care and rehabilitation plans and coordinate across services.
• Support CGA implementation and contribute to frailty hub development and service improvement.
