Job summary

The Royal United Hospitals Bath NHS Foundation Trust invites applications for an RUH@Home physiotherapist/occupational therapist role. Working as part of the RUH@Home multidisciplinary team, you will manage a delegated caseload and deliver acute hospital-level care, rehabilitation and enablement in patients' homes.

You will conduct structured follow-up assessments, develop and progress rehabilitation plans, monitor progress, identify risks, and escalate to a registered therapist as needed.

Responsibilities

• Manage and prioritise a delegated caseload, working autonomously in patients' homes within agreed competencies.
• Undertake protocol-led step-down assessments for patients previously assessed by a hospital Physiotherapist or Occupational Therapist, using the approved assessment proforma.
• Gather, interpret and record information on mobility, transfers, activities of daily living, rehabilitation needs, equipment, environmental factors and risk.
• Deliver and progress delegated rehabilitation, reablement and exercise programmes in line with agreed goals and treatment plans.
• Monitor clinical and functional progress, recognise deterioration or variance from the expected pathway, and elevate promptly to the registered therapist or wider multidisciplinary team.
• Support admission avoidance, timely discharge, prevention of deconditioning and reduction in ongoing care needs.
• Contribute to multidisciplinary handovers, care planning, discharge planning, service data collection and clinical documentation.
