Job summary

Kent Community Health NHS Foundation Trust is seeking an HCPC-registered Occupational Therapist or Physiotherapist to join our urgent care service. You’ll assess, plan, implement and evaluate rehabilitation for patients in community settings, managing a caseload and supporting innovations such as Virtual Wards.

The role requires autonomous working within a multidisciplinary team, with emphasis on patient-centred care and complex case management.

Qualifications

• HCPC registration is required.
• Experience in a health improvement environment.
• Full UK/EU driving licence.
• Management or supervisory qualification or experience of study.
• Working towards Master level study.
• Evidence of Specialist qualifications relevant to area of practice (e.g. non-medical prescribing; mentorship, coaching, clinical expertise).
• Health-related degree and evidence of relevant post graduate development at Level 6.
• PTLLS/DTLLS, PGCE or Certificate in Education.

Responsibilities

• Drive to perform duties where driving is a requirement and ensure access to a full UK driving licence.
• Support patients with complex needs through assessment, plan, implement and evaluate care.
• Contribute to service development, quality improvement, and operational objectives.
• Work with multidisciplinary teams to deliver rehabilitation and enablement in community settings.
