Job summary

Central London Community Healthcare NHS Trust is seeking a Physiotherapist to provide a comprehensive physiotherapy service to residents of West Hertfordshire within the bed-based unit, working autonomously and in line with professional standards and guidelines.

You will assess and treat your own caseload, supervise Rehabilitation Assistants and physiotherapy students, and collaborate with acute and community teams to facilitate referrals and service development across diverse settings.

Qualifications

• To assess the functional needs of patients and develop intervention plans to improve function.
• To implement treatment plans for patients with neurological, musculoskeletal, age-related conditions and other pathologies.
• To undertake comprehensive assessment, planning and treatment using clinical reasoning.
• To communicate information and discharge plans across agencies with informed consent.

Responsibilities

• Assess the functional needs of patients and develop intervention plans to improve function and reduce disability.
• Develop and implement treatment plans for patients with varied pathologies under supervision where required.
• Undertake comprehensive assessment and treatment using clinical reasoning.
• Communicate medical information and discharge plans across GP, social services, and other agencies with informed consent.
• Supervise Rehabilitation Assistants and Physiotherapy students with guidance from Senior Therapists.
