Job summary

Dudley Group NHS Foundation Trust is seeking a Community Nurse Practitioner to deliver high-quality patient care within the Community Hub. The role emphasises autonomous practice, advanced clinical skills, and leadership to develop new ways of delivering services across localities.

As part of the Community Urgent Care Response Team, you will perform rapid clinical assessments, diagnose and treat, and, where required, prescribe (V300).

Qualifications

• Experience of service development and clinical leadership.
• Autonomous practitioner with advanced clinical practice skills.
• Prescriber (V300) preferred.

Responsibilities

• Assess health and clinical needs and conduct autonomous consultations.
• Diagnose, treat, refer to other services and discharge patients.
• Respond to urgent care/crisis within a 2-hour target and deliver high‑quality care.
