Job summary

East Cheshire NHS Trust is seeking a motivated staff nurse to join our Community Nursing Team covering Handforth and surrounding localities. The role focuses on autonomous home nursing for the elderly in the CHAW area, coordinating care within a multidisciplinary team.

The post is 18.75 hours per week, with shifts typically 9am–5pm and potential for varied patterns. Applicants should be prepared for travel and lone-working in the community.

Qualifications

• Registered Nurse to degree / diploma level
• Current registration with NMC (Nursing and Midwifery Council)
• Evidence of recent professional development in an up-to-date portfolio

• Provide autonomous holistic nursing care and assessment in the community
• Develop, implement and evaluate care plans within the nursing team
• Delegate tasks appropriately and ensure safe, patient-centred care
