Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking Community Staff Nurses for posts in Alresford and Colden Common. • Ability to record care plans and holistic assessments to the required standards.

Responsibilities

• Manage, coordinate and delegate work within a defined community caseload.
• Ensure referrals are dealt with according to clinical need and holistic assessments.
• Record care plans in line with Trust and Professional standards and maintain patient safety.
• Support timely discharge to the community and enable re-enablement and rehabilitation.
