Job summary
Cambridgeshire and Peterborough NHS Foundation Trust is seeking an autonomous practitioner to work within an integrated multi-professional neighbourhood team. You will manage your own caseload and assess patients with complex presentations using specialist clinical reasoning.
You will provide high-quality nursing interventions in patients' homes, supervise junior staff and students, and coordinate with the MDT.

- Assess patients with complex and multiple pathologies using specialist clinical reasoning.
- Provide holistic nursing interventions and administer care in patients' homes, including lone working.
- Supervise junior staff, students and non-registered staff, ensuring competencies.
- Coordinate with MDT, contribute to risk assessments, and document changes to care plans.
