Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking Community Staff Nurses for posts in Alresford and Colden Common. You will provide high-quality care across home and clinic settings, working with a high degree of autonomy and professional accountability within a supportive team.
Responsibilities include managing a defined community caseload, coordinating referrals, and developing holistic care plans in line with Trust standards.

Successful post holders will work with a high degree of autonomy and professional accountability, be committed to working within a generic competency framework enabling you to have the clinical skills to work as a valued member of the team. You will utilise a skill set in wound care, palliative care, end of lifecare as well as many others. Post holders will have responsibility for managing ,co-ordinating and delegating work ensuring referrals are dealt with according to clinical need and holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
These are part or full time posts for 12 month fixed term contracts, Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload for a group of GP Practices as part of the Integrated Care Team.
The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities
