Job summary

Cornwall Partnership NHS Foundation Trust is seeking a caring community nurse to join the Stratton Community Nursing Team, based at Stratton Hospital, covering Bude and surrounding areas. This fixed-term post supports maternity cover with flexible full- or part-time hours.

Role includes autonomous nursing in patients' homes, travel across the county, and close collaboration with GP practices, therapists and voluntary organisations. Interview date: 21 September 2026.

Qualifications

Registered Nurse (Adult) with current NMC registration.
Mentoring qualification or willingness to work towards mentoring updates.
Post-registration diploma or degree in nursing or equivalent education.

Responsibilities

Assess, plan, implement and evaluate care in patients' home environments without direct supervision.
Work with patients and carers to provide support, advice and informed choice.
Provide effective, evidence-based nursing care in line with policies and guidelines.
Support locality teams by moving to where service provision demands.
