Job summary

Cornwall Partnership NHS Foundation Trust is seeking a caring Community Nurse (Band 5) to join the Stratton Community Nursing Team, covering Bude area. This fixed-term post supports maternity cover with 9 months duration and flexible full/part-time hours.

Based at Stratton Hospital, the role involves home visits, collaboration with GP practices, therapists and social care, and autonomous practice with ongoing professional development in a supportive NHS environment.

Qualifications

Job overview

We are looking for a caring, motivated and dynamic community nurse to join our friendly, professional team.

This is a fixed term contract to cover maternity leave. Expected duration of this post is 9 months. Full or part time hours available.

You will be part of the Stratton Community Nursing Team and you will be based at Stratton Hospital, covering Bude and the surrounding area. We cover Stratton and Neetside GP practices within the local PCN. Your role will require close collaboration with GP's, health and social care colleagues including therapists and voluntary organisations. We would like you to possess commitment to team working and embrace innovative delivery of excellent standards of care. You will need to have a real enthusiasm for community nursing as an autonomous practitioner and a desire for ongoing professional development.

You should also have an understanding of the changing climate of care delivery in the community.

Shift patterns are 0800 - 1600 and 1400 - 2200.

Main duties of the job

This is a community-based role that requires frequent travel to various locations throughout the county including areas that may not be accessible by public transportation. Successful candidates will need to demonstrate how they will manage these travel requirements.

Following an initial induction and ongoing clinical supervision, you will be responsible for: Assessment, planning, implementation and evaluation of care, without direct supervision, to patients, carers and relatives within their home environment. The aim is to enable them to enjoy maximum independence and optimum quality of life.

To work in partnership with patients and carers to offer support and advice and promote informed choice.

To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines.

To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.

Education and Qualifications
  • Registered Nurse (Adult)
  • Evidence of recent relevant professional development
  • To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
  • Current Professional Registration - NMC
  • Hold a post-registration diploma or be working towards a degree
Skills and Aptitude
  • IT skills to include use of Word, Excel, email, internet, recording systems
  • The ability to provide skilled, evidence based nursing care to a wide range of patients
  • Ability to deal with patients and carers who are under stress.
  • Awareness of and adherence to policies including lone worker and manual handling policies
  • Current knowledge of developments in nursing and healthcare in general.
