Job summary

Cornwall Partnership NHS Foundation Trust, Bodmin, is recruiting a Community Staff Nurse (Band 5) for the Bodmin & Three Harbours Community Team. The post is permanent, full time (37.5 hours) with evenings, weekends and bank holidays.

You will assess, plan, implement and evaluate care in patients' homes across Bodmin, Par, Lostwithiel and surrounding areas, working with GP, social care and voluntary partners to deliver high-quality, compassionate community nursing.

Qualifications

• Nurse (Adult) registered with the NMC and current professional registration.
• Post-registration qualification or degree and commitment to CPD.
• Mentoring qualification or willingness to undertake mentoring training.
• Demonstrated recent professional development relevant to community nursing.

Responsibilities

• Assess, plan, implement and evaluate care in patients' homes with minimal supervision.
• Work with patients and carers to promote informed choice and independence.
• Collaborate with GP, social care, therapists and voluntary organisations in a flexible, team-based approach.
