Job summary

Northumbria Healthcare NHS Foundation Trust in Hexham is inviting applications for a District Nursing Staff Nurse (Band 5) to join our community team. The role covers home visits, clinics and collaboration with GPs and care homes to deliver patient-centred care.

Qualifications

• State Registration on Part 1 of the 3 part NMC register.
• Additional clinical qualifications or equivalent derived through experience.
• Newly qualified staff will be required to undertake the Trust preceptorship programme within the first year in post.

Responsibilities

• Assess, plan, implement and evaluate care for a diverse group of patients with a wide range of conditions and health needs.
• Develop and deliver programmes of health and social care.
• Provide care and support to patients with terminal illness and their families including bereavement support.
• Perform skilled nursing care including wound management, palliative care and catheterisation.
• Interpret results and communicate with patients and carers about care.
• Maintain high standards of professional practice and accountability.
• Work autonomously and as part of a team within district nursing service.
