Job summary

Woolston and Chartwell Partnership in Southampton seeks an enthusiastic Advanced Clinical Practitioner (ACP) to join a 6-partner practice supported by a full MDT including nurses, HCAs, pharmacists and social prescribers.

The ACP will work autonomously within scope, managing minor and acute care, participating in care planning, and contributing to same-day demand and urgent care needs across face-to-face, video, telephone, and home visits.

Qualifications

• Registered healthcare professional with current professional registration (NMC/HCPC).
• Experience with team-based care and MDTs.

Responsibilities

• Provide care for patients presenting with minor and acute illness and injury in collaboration with GPs.
• Assist same day demand and acute care needs via in-person, video, telephone and e-consultations and home visits.
• Develop and implement evidence-based treatment plans and prescribe medications independently within scope (desirable).
• Identify red flags and refer patients to secondary or specialist services as needed.
• Maintain accurate patient records using electronic systems.
• Promote health, screening and preventative care.
