Job summary

East of England Community Health & Care NHS Trust is recruiting an Advanced Clinical Practitioner to provide autonomous clinical assessments in community and inpatient settings across Norfolk.

You will lead in a role that supports patients at home or in community inpatient units, requiring travel across Norfolk and a driving licence. The post emphasizes clinical autonomy, prescribing under PGDs, and strong MDT collaboration.

Qualifications

• Act as an advanced practitioner with clinical competence beyond conventional roles.
• Lead a culture open to change, new ideas and innovation.
• Undertake comprehensive clinical consultations with patients as an autonomous practitioner.
• Make direct referrals within locally agreed pathways, guidance and protocols.
• Prescribe, supply and administer medicines as indicated to address patient needs within PGDs.
• Ensure accurate and complete documentation of each patient contact.
• Work as part of a multi-disciplinary and multi-agency team to meet patient needs.
• Utilise critical thinking and problem-solving skills including reflection.

Responsibilities

• Collaborate with multidisciplinary teams to meet patient needs.
• Apply critical thinking and reflection to everyday practice.
