Job summary

Lincolnshire Partnership NHS Foundation Trust in Lincolnshire is recruiting a Keyworker Support Worker to assist autistic children and young people and those with learning disabilities, aligning to locality areas. You will support care planning, engage with families, and contribute to multi-agency care.

The role requires lived or learnt experience of Autism/LD, and offers varied duties across direct work, visits and safeguarding responsibilities within an NHS framework.

Qualifications

  • GCSE English & Maths (or equivalent)
  • NVQ Level 3 in Health & Social Care, Childcare, Education, or related field, or equivalent
  • Training in autism, learning disability, trauma-informed practice or de-escalation or other relevant training

Responsibilities

  • Support the delivery of person-centred care plans for children, young people and families through direct work, including visits, interventions and ongoing support.
  • Work under the guidance of a Keyworker to carry out delegated tasks safely and effectively, contributing to coordinated care.
  • Build positive, trusting relationships and ensure the voice of the child, young person and family is heard and reflected in care planning.
  • Support families to navigate health, education and social care systems, helping them understand processes and access appropriate services.
  • Work collaboratively with multi-agency partners, attending meetings and sharing relevant information to support joined-up care.
  • Assist with triage, screening visits and gathering information to inform assessments and care planning.
  • Identify, respond to and report safeguarding or risk concerns in line with Trust policies.
  • Maintain accurate and timely records, contributing to monitoring outcomes and service evaluation.
  • Engage in supervision, training and service development, supporting continuous improvement and high-quality practice.
