Job summary

West Sussex County Council is recruiting a Transforming Care Keyworker to support children and young people aged 0-25 with autism and/or learning disabilities and complex mental health needs. The role is countywide with a base in Chichester, Littlehampton or Horsham, requiring independent travel and a full driving licence.

You will work with families and partners to deliver personalised, integrated support within the national Keyworker Project, partnering with NHS England and the Sussex

Qualifications

• Ability to always put the child first to support positive outcomes.
• Strong communication with professionals, families, children and young people.
• Ability to interpret complex information to identify family needs and plan support.
• Knowledge of safeguarding procedures and child protection.
• Understanding of neurodevelopmental pathways relevant to West Sussex.
• Full, valid driving licence and willingness to travel countywide.
