Job summary

Lincolnshire Partnership NHS Foundation Trust in Lincolnshire is recruiting a Keyworker Support Worker to assist autistic children and young people and those with learning disabilities, aligning to locality areas. You will support care planning, engage with families, and contribute to multi-agency care.

The role requires lived or learnt experience of Autism/LD, and offers varied duties across direct work, visits and safeguarding responsibilities within an NHS framework.

Qualifications

• GCSE English & Maths (or equivalent)
• NVQ Level 3 in Health & Social Care, Childcare, Education, or related field, or equivalent
• Training in autism, learning disability, trauma-informed practice or de-escalation or other relevant training

Responsibilities

• Support the delivery of person-centred care plans for children, young people and families through direct work, including visits, interventions and ongoing support.
• Work under the guidance of a Keyworker to carry out delegated tasks safely and effectively, contributing to coordinated care.
• Build positive, trusting relationships and ensure the voice of the child, young person and family is heard and reflected in care planning.
• Support families to navigate health, education and social care systems, helping them understand processes and access appropriate services.
• Work collaboratively with multi-agency partners, attending meetings and sharing relevant information to support joined-up care.
• Assist with triage, screening visits and gathering information to inform assessments and care planning.
• Identify, respond to and report safeguarding or risk concerns in line with Trust policies.
• Maintain accurate and timely records, contributing to monitoring outcomes and service evaluation.
• Engage in supervision, training and service development, supporting continuous improvement and high-quality practice.

Keyworker Support Worker

Job overview

Following a recent service review, we are delighted to introduce Keyworker roles as part of our enhanced non-clinical offer. We will be recruiting to one position, aligned to locality areas:

Grantham – Sycamore Centre

You will support autistic children and young people, and those with a learning disability, who may be at risk of hospital admission. Working alongside a Keyworker, you will help deliver personalised support through visits, interventions and ongoing engagement with families.

You will build positive relationships, support families to navigate complex services, and ensure the voice of the child or young person is heard and reflected in their care. The role includes contributing to multi‑agency working, maintaining accurate records, and supporting coordinated, community‑based care.

Person specification

Qualifications

• Good general education (e.g., GCSE English & Maths or equivalent)
• Willingness to complete all mandatory training, including Breakaway and safeguarding
• Commitment to ongoing personal and professional development
• NVQ Level 3 in Health & Social Care, Childcare, Education, or related field, or equivalent
• Training in autism, learning disability, trauma-informed practice or de‑s‑calation or other relevant training for this post

Experience

• Previous relevant experience working with Autistic people and/or those with Learning Disabilities
• Previous relevant experience with Children and Young people and their families
• Knowledge of risk assessment principles
• Experience in providing support for people in the community
• Knowledge and understanding of safeguarding procedures
• Knowledge and understanding of Mental Capacity and Best Interest
• Experience supporting individuals who may be distressed, dysregulated or present with challenging behaviour
• Experience maintaining accurate written or electronic records
• Experience in providing support to autistic children and young people, and/or those with a learning disability, who are risk of hospitalisation
• Experience of multi‑agency working (health, education, social care, voluntary sector)
• Experience supporting CYP and families to express their views and be heard
• Experience in lone working

Skills

• Ability to communicate sensitively and effectively with CYP and families, including those in distress
• Ability to adapt communication to age, developmental needs, autism/LD profiles and communication differences
