Job summary

Norfolk County Council is recruiting a full-time Assistant Practitioner to join the Northern Mental Health Social Care Team, covering North Norfolk. The role involves Care Act assessments, support planning, reviews, and duty team coverage within a hybrid working model.

The successful candidate will work with partners to promote independence, using a Living Well agenda and flexible working arrangements, including two days per week in the community or office setting.

Qualifications

  • Knowledge of the Care Act (2014) and Mental Capacity Act (2005).
  • Strong time management and IT skills.
  • Experience in social care or similar environment preferred.

Responsibilities

  • Undertake Care Act Assessments, Support Planning and Reviews.
  • Cover Duty Team responsibilities as needed.
  • Collaborate with partners (NHS, Trusts, stakeholders) to promote independence.

Assistant Practitioner | 11212 | permanent contract | 37 hours per week | £33,119 to £34,811 per annum | Grade G | Norwich

An exciting opportunity has arisen for us to recruit a full time Assistant Practitioner to join the Northern Mental Health Social Care Team, covering the North Norfolk area as well as Mental Health Duty Team.

We have embraced our new Community Social Work model, Human Learning Systems, in looking at new and creative ways to prevent, reduce and delay care and support needs. Early intervention, creativity and utilising community assets are key, as is improving our community led support offer that will include engaging with people in public spaces. This is an exciting time to join Norfolk Adult Social Care, so if you share our vision, we’d like to hear from you.

You will be joining a passionate, forward thinking, supportive and high performing team. You will undertake Care Act Assessments, Support Planning, Reviews and take part in covering our duty team whilst working collaboratively with partners including Norfolk and Suffolk Foundation Trust and various stakeholders to promote independence.

We are seeking an enthusiastic community Assistant Practitioner to join our highly motivated, innovative team of Practice Consultants, Social Workers, Assistant Practitioners and Living Well Officers. It is an exciting time to join our team of dedicated and passionate workers in embracing our Living Well agenda and supporting people to be independent, resilient and well.

The successful candidate will have knowledge and a level of experience of the Care Act (2014), Mental Capacity Act (2005) Mental Health Act (1983) and Safeguarding vulnerable adults, as well as excellent time management and IT skills. All new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures.

We operate a hybrid model of working, which includes working from home and from our office base at Hellesdon Hospital. There are also opportunities to work from other Norfolk County Council bases.
