Job summary

The Vale Locality Mental Health Team is seeking a motivated social worker to support adults with severe and enduring mental health needs under the Mental Health (Wales) Measure in the Vale of Glamorgan. You will work with health colleagues to assess, formulate and review integrated care and support plans, promoting independence and resilience.

Grade 8/9 posts require experience with complex cases; AMHP status is an asset.

Qualifications

• Experience of working with adults who are vulnerable through illness, disability or age-related frailty.
• Assessment of Adult Service users and carers
• Working in partnership with statutory/voluntary organisations to promote the independence of service users.

Responsibilities

• Undertake assessments and formulate and review integrated care and support plans.
• Assist the Local Authority in discharging statutory duties under the Mental Health Act (if AMHP).
