Job summary

Adult Social Care in Powys is seeking a Social Worker – Mental Health to support people with complex mental health needs and their carers. You will assess, plan and monitor care, conduct safeguarding investigations and work with health professionals and agencies to provide joined-up support.

The role emphasizes promoting independence, wellbeing and social inclusion, with close partnership working and robust case management across a diverse caseload.

Qualifications

• Strong written and verbal communication skills.
• Good IT and record-keeping skills.
• Ability to organise, prioritise and manage a caseload.
• Strong assessment, risk management and safeguarding skills.
• Ability to manage crises, conflict and challenging situations effectively.
• Ability to build and maintain effective partnerships with individuals, carers, professionals and other agencies

Responsibilities

• Assess and manage the complex needs and risks of people experiencing mental health difficulties.
• Manage an allocated caseload, completing assessments, care planning, monitoring and reviews.
• Undertake safeguarding investigations and help protect vulnerable adults from harm.
• Work collaboratively with individuals, carers, health professionals and other agencies to provide joined-up support.
• Complete Mental Capacity Act assessments, Best Interests decisions and AMHP duties, where appropriate.
• Maintain accurate case records and professional reports, while promoting independence, choice, wellbeing and social inclusion.
