Job summary

Adult Social Care in Powys is seeking a Social Worker – Mental Health to support people with complex mental health needs and their carers. You will assess, plan and monitor care, conduct safeguarding investigations and work with health professionals and agencies to provide joined-up support.

Responsibilities

• Assess and manage the complex needs and risks of people experiencing mental health difficulties.
• Manage an allocated caseload, completing assessments, care planning, monitoring and reviews.
• Undertake safeguarding investigations and help protect vulnerable adults from harm.
• Work collaboratively with individuals, carers, health professionals and other agencies to provide joined-up support.
• Complete Mental Capacity Act assessments, Best Interests decisions and AMHP duties, where appropriate.
• Maintain accurate case records and professional reports, while promoting independence, choice, wellbeing and social inclusion.

You will receive an honorarium of £3000 per year, paid monthly for the AMHP work you undertake

About the role:

The Social Worker – Mental Health role supports people with complex mental health needs and their carers across Powys. Responsibilities include assessment, risk management, safeguarding, care planning, Mental Capacity Act work and managing a caseload.
