Job summary

Cardiff Council's Inclusion Service is seeking an ALN Transition Worker (Placements) to join the Education Service. You will work with parents and carers to ensure smooth transitions into specialist provision for children with additional needs and disabilities, coordinating with schools and partner agencies.

The role is full-time (37 hours per week across 52 weeks, permanent) and welcomes applications in both English and Welsh. A highly organised and proactive communicator is essential.

Qualifications

• Excellent communication skills both verbal and written.
• Highly organised, motivated and passionate.
• Ability to manage difficult conversations as part of disagreement resolution.

Responsibilities

• Work with parents/carers to ensure smooth transition into specialist provision.
• Coordinate with Inclusion Team and partner agencies to support progression.
