Job summary

NHS Jobs in Tunbridge Wells is seeking a Senior Clinical Pharmacist to join the Primary Care Network. You will work within a multidisciplinary team to optimise medicines on transfer of care and carry out face-to-face reviews for patients with polypharmacy and frailty.

You will lead on quality improvement, clinical audit, and antibiotic stewardship while supporting Extended Hours sessions and weekend work as needed. Strong pharmacotherapy skills are essential.

Responsibilities

• Work as part of a multidisciplinary team to manage medicines on transfer of care.
• Perform face-to-face medication reviews of patients with polypharmacy, frailty or multiple comorbidities.
• Carry out reviews for long-term conditions within competency (e.g., asthma).
• Lead Improved Access and Extended Hours sessions, including some weekend work.
• Provide leadership on quality improvement and clinical audit, medicines safety and antibiotic stewardship.
• Contribute to practice financial stability through optimisation of medicines targets in QOF.
