Job summary

Derbyshire Community Health Services NHS Foundation Trust is seeking an enthusiastic Advanced Therapy Support Worker for the Early Supported Stroke Discharge (ESSD) team. The role covers High Peak and North Dales, delivering home-based stroke rehabilitation in line with the ESSD model.

You will assess needs, prescribe equipment, and evaluate safety while working with therapists to enable early hospital discharge and safe recovery at home.

Qualifications

• Travel across locality and access to a suitable vehicle for business purposes.

Responsibilities

• Work with Qualified Therapists in assessment and treatment of stroke patients as part of ESSD.
• Carry own caseload of patients and progress intervention programmes within set parameters.
• Work alone in patient residences or clinics, under supervision when required.
• Communicate effectively with patients and carers to maximise rehabilitation potential.
