Job summary

Derbyshire Community Health Services NHS Foundation Trust is launching an Early Supported Stroke Discharge (ESSD) service in High Peak and North Dales. We are seeking an Advanced Therapy Support Worker to join a multidisciplinary team delivering evidence-based stroke rehabilitation in patients' homes and clinics, with supervision.

Travel across the locality is required; the role supports hospital discharge pathways and patient-centred care within the ESSD model.

Qualifications

• Experience in stroke rehabilitation or community health settings.
• Ability to work in a multidisciplinary team.
• Excellent communication and organisational skills.

Responsibilities

• Work with Qualified Therapists in the assessment and treatment of stroke patients.
• Carry own caseload of patients and progress/modify intervention programmes within parameters set by the therapists.
• Work alone in the patient’s home or clinic, under supervision but often without a qualified therapist.
• Communicate effectively with patients and carers to maximise rehabilitation potential.
• Travel independently across the locality to cover High Peak and North Dales as required.
